Quality Insights, Mountains of Hope Cancer Coalition, WV Health Information Network, WV Center for End of Life Care and the Hospice Council of WV are the founding partners of the WV Serious Illness Collaborative. This collaborative began meeting in 2021 and included the chair and some members of the State Advisory Coalition on Palliative Care. Plans were made to continue the important work of this coalition after it sunsetted in December 2021.

The mission of the collaborative is:-

To empower West Virginians who have a serious, chronic, or advanced illness, those providing care for them and their families to ensure patient-centered care that aligns with the patient’s goals, values and preferences and provides the best quality of life.

The WV Serious Illness Collaborative has been supported by the Center to Transform Advanced Care (C-TAC) and the Center to Advance Palliative Care (CAPC) as well as other state coalitions that have similar missions.

Current goals of the WV collaborative are to increase awareness and access to palliative care; promote advance care planning; and to support payment reform for palliative care to ensure equitable access.

In 2020, Governor Justice proclaimed that November was Palliative Care Awareness Month.
